SITE VISIT:

We strongly encourage subcontractors to visit the construction site prior to submitting a proposal.  A site visit can be set-up with the Project Manager or Estimator.

 GENERAL NOTES:

In regards to the above referenced project, Penzel is the General Contractor with work progressing on schedule.

 

The Project consists of General Construction and Renovation of the MRI Suite at Southeast Missouri Hospital in Cape Girardeau, Missouri.

The Work of Project is defined by the Contract Documents and consists of the following:

1.    Renovation of existing MRI suite with adjacent support spaces. Additional spaces are Included for above-ceiling medical gas piping and public restroom renovations.

Working Conditions of Existing Building:

1.    Maintain portions of existing building affected by construction operations in a weathertight condition throughout construction period.  Repair damage caused by construction operations.

2.    Limit use and operation within existing facilities to areas indicated for Construction work and as required to perform Work. Other areas within facility shall not be disturbed or disrupted.

3.     Perform Work so as not to interfere or inconvenience public, staff and Owner’s Operation.

4.     Maintain and keep clear all required fire exit ways throughout facility within and in vicinity of construction areas. Coordinate alternate temporary egress routes with Owner and local fire authority.

5.     Smoking is absolutely prohibited within facilities or on Owner’s property.

6.     Audio devices and radios are prohibited; except that two-way radios, and their use within occupied facilities shall be limited so not to disrupt occupants.

7.     Cafeteria and dining areas in existing facility may not be used by construction personnel without Owner’s consent.

 

8.     T-shirts or other clothing with derogatory depictions, language, and/or slogans regarding alcohol, drugs, race or sexual in nature, shall not be worn on premises.

9.     Derogatory language regarding race, sexual or religious in nature, shall not be used on premises. 

10.  Make every effort to keep noise to a minimum in construction operation. Jackhammers shall not be permitted for use within the existing building without the Owner’s consent.

The Project is subject to the requirements of Section 292.675, rsmo, which requires all on-site employees to complete, a ten (10) hour course in construction safety and health approved by the Occupational Safety and Health Administration (“OSHA”) or a similar program approved by the Missouri Department of Labor and Industrial Relations & have card on-site.

 Penzel Construction Company, Inc., the Owner, and all other parties as required by the contract are added as primary additional insured under Subcontractors Comprehensive General Liability policy.  For additional information see “project insurance requirements” on our web site.

 

Subcontractors are required to adhere to all Penzel and OSHA Safety Policies.  The Penzel Standard Form Contract will be utilized without modification for all subcontractors.  A copy of the Penzel Safety Policy or Standard Form Contract is available upon request.  The OSHA Safety and Health Requirements Manual is available on the OSHA web site at www.osha.gov.   SoutheastHEALTH requires that everyone working on site register with RepTrax and receives orientation for the Hospital’s Interim Life Safety Measures which includes taking the electronic ILSM competency prior to starting work.

 

Subcontractors are responsible for clean up of material and waste associated with their scope of work.  Clean-up will be coordinated with the on-site superintendent and will be performed on a daily basis.  If the subcontractor is deemed unresponsive to this requirement, Penzel will perform the activity at the expense of the subcontractor.
